RECORDS REQUEST DRAFTS - NOT SENT

1. CATHOLIC CEMETERIES ASSOCIATION / SAINT LAWRENCE OFFICE
Telephone: (203) 624-3980. https://ccacem.org/cemetery/saint-lawrence-cemetery/
We are compiling an educational archive for Saint Bernard Cemetery, 520 Columbus Avenue, New Haven. Please identify the custodian and date coverage of its interment registers, alphabetical burial index, lot-owner cards, deeds, grave-opening records, burial returns, removal/disinterment permits and section/lot maps. Is a digital export or supervised scan of the complete historical register available? Please identify restrictions and quote any copying fees before work begins. Please preserve original section, avenue, lot and grave wording, including renumbering keys. We are not asserting that the cemetery's records were lost.

2. NEW HAVEN MUSEUM / WHITNEY LIBRARY
Please quote digital copies of MSS 23, Box 2 folders D-F, Saint Bernard headstone inscriptions pp. 1157-1411, including all title sheets, section headings and maps, plus the relevant Box 3 name-index entries. Reference copy: Ref F104.N6 H244; Hale cemetery code 215-6. Please also identify Saint Bernard entries in MSS 80 Box 26, including Civil War burial lists and World War I casualties returned to New Haven. Please confirm image reproduction terms. Finding aids: https://www.newhavenmuseum.org/wp-content/uploads/2013/06/MSS-23.pdf and https://www.newhavenmuseum.org/wp-content/uploads/2013/06/MSS-80.pdf

3. NEW HAVEN VITAL STATISTICS
For [full deceased name], [date or date range], please identify the death record and any retained burial or removal permit naming Saint Bernard Cemetery in New Haven. Please advise on historical search access, required identification and copying fees. For a larger project, is an index searchable by cemetery available? Do permit records identify undertaker, burial date, section or lot? We will follow the current application instructions at https://www.newhavenct.gov/government/departments-divisions/vital-statistics-division/marriage-license . Do not include Social Security numbers in material intended for public publication.

4. CONNECTICUT IRISH-AMERICAN HISTORICAL SOCIETY / LIBRARY
Please advise how to consult Neil Hogan's Early New Haven Irish and their final resting places and the Saint Bernard Civil War veteran research discussed in The Shanachie. Are the underlying inscriptions, plot plan, veteran roster and photographs available for educational research? Please identify source dates and any permission needed to reproduce scans or photographs. We would like to distinguish relocated stones from documented reinterments from the Old Catholic cemetery.

5. PHOTOGRAPH OR CLIPPING CONTRIBUTOR
We are documenting [record ID / person's name]. May SGC include your [specific photograph or clipping] in its public educational archive and downloadable ZIP? Please provide the original image, photographer, date, cemetery, section/lot/grave if known and the reuse terms you authorize. Please include an overall marker image and neighboring-marker context as well as readable close-ups. No permission has yet been granted.
